    • DECLARATION OF RESIDENCY FORM

      A basic form to record the parent/guardian information and the circumstances related to their living situation.   This will formalize the process related to an indication of living in transition with both parent/guardian and homeless liaison signatures.   If you are familiar with using Google Docs, "Make a Copy" and rename the file to suit the situation.  I tend to use "MV" then the last name of the family who we are recording the information.  If you are not familiar with Google Docs, you can always print this and hand write the information as necessary.

    • LEA GENERAL INTENT TO COLLABORATE

      This form will be used by our offices for each school district to agree to collaborate with the Region Program Coordinator to serve the best interest of the students in transition from your school district.  This just formalizes the agreement to share information and support in an effort to serve the students and their families.

    • MCKINNEY-VENTO DISTRICT UPDATE FORM

      Use this form in order to follow up with parents or guardians in relation to their living situation at the end of the school year or during the summer months.  This will keep your records accurate and provide the best services for the students.

    • MCKINNEY-VENTO ELIGIBILITY DISPUTE FORMS - LIAISON AND PARENT APPEAL

      This is a sample form that could be used should a determination be made that a student(s) are not deemed eligible for homeless status and resources.  Liaisons should use the first part of the form in conjunction with either a supervisor or the superintendent and offer the appeals paperwork as part of the process.  This form need not be shared with me unless you wish to have my review of the circumstances related to the situation.  You can either keep electronically or print out as necessary.

    • MCKINNEY-VENTO IDENTIFICATION OF DISPLACED FAMILIES/APPLICATION FOR SERVICES FORM

      It is recommended that you use this form or something similar to help your registrars in identifying families who are displaced with students in transition. This will help move these students forward to the District Homeless Liaison for eligibility purposes. This form can be modified to fit your district by making a copy of it in Google Docs.
